Is it time to revisit the Boston Carpal Tunnel Questionnaire? New insights from a Rasch model analysis

Background The Boston Carpal Tunnel Questionnaire (BCTQ) is a patient-reported outcome measure (PROM) used to symptom severity and function in carpal tunnel syndrome (CTS). Despite its wide use, investigation of measurement properties using modern psychometric methodologies limited. Methods Completed BCTQ data collected routinely the Canterbury clinic was investigate structural validity through application Rasch model analytic approach. Results A total 600 patients with electrodiagnostically confirmed CTS their right hand were randomly selected from database analyzed. Mean age 48.8 y, 73% women. Initial analysis showed that 19 items could not be reliably added up form single linear construct. All subsequent analyses done by subscale only. Symptom Severity Subscale (SSS) displayed large amount local dependence. This accommodated creation four clinically derived testlets, allowing for ordinal SSS raw score transformed measure. Functional Status (FSS) number issues regarding integrity. These include scale item fit, targeting, differential functioning, dimensionality. Conclusions study shows generated across all psychometrically valid, FSS subscales should treated separately. We propose modified scoring system SSS, resulting can future existing datasets.
